- After reviewing the information, fill out the SR&R Parent Acknowledgement Form using this link we have provided. The form must be filled out one time per student.

- Family Life Education (FLE) Opt-Out Forms
- Optional: If you wish to opt your child out of all or some of the Family Life Education lessons, please complete this form and return it prior to FLE instruction. These forms are available by grade level and in multiple languages.
